Diazoxide for the treatment of hypoglycaemic dumping syndrome.

چکیده
Dumping syndrome is a commonly recognized complication after gastric and oesophageal surgery. It is characterized by postprandial early gastrointestinal and vasomotor symptoms followed by late hypoglycaemia. Early dumping symptoms include abdominal pain, bloating, nausea, vomiting, flushing, tachycardia and hypotension. Late dumping symptoms which occur 1–3 h after food are thought to be due to excess insulin secretion that results in hypoglycaemia. Management of hypoglycaemia in dumping syndrome can be challenging and the therapeutic options are dietary modifications, acarbose, octreotide and diazoxide all providing varying degrees of symptomatic relief. Dumping syndrome is likely to become more prevalent due to the pandemic of obesity and associated use of bariatric surgery. Diazoxide has been occasionally used with some success in post-bariatric surgery dumping syndrome but there are no reports of its effective use in treating hypoglycaemia associated with primary gastric surgery. We describe three patients with hypoglycaemic dumping syndrome after different gastric surgeries that were successfully treated with diazoxide.

Pasireotide and octreotide in the treatment of severe late dumping syndrome
Hypoglycemia due to late dumping is a significant problem postoesophagectomy but may not always be diagnosed sufficiently early. It can be difficult to treat and may severely compromise quality of life. The combination of diazoxide and octreotide or more probably pasireotide may transform the patient's life and should be considered in all problematic cases.
متن کاملManagement of prolonged hypoglycaemia in Beckwith's syndrome.
A new-born baby with Beckwith's syndrome had severe hypoglycaemia, and was treated with glucagon and prednisone. Treatment was stopped at 1 month of age and his blood glucose levels were satisfactory for the ensuing 6 weeks. However, at 10 weeks of age, he had a hypoglycaemic convulsion and required large doses of diazoxide and prednisone for the next 2 months to maintain the blood glucose abov...

Results Five infants were reported. Hypoglycaemic seizure was the commonest presenting feature. All had detectable insulin level (>5 mU/l) during hypoglycaemic episodes and high glucose requirement (>10 mg/kg/min). Three patients were found to have genetic mutation associated with CH. Patient 1 had hyperammonaemia hyperinsulinism syndrome and corresponded missense mutation of GLUD1 gene. For pa...
متن کاملNon-islet Cell Tumour Hypoglycaemia (NICTH) in Malignant Mesothelioma: Case Report.
Non-islet cell tumour hypoglycaemia (NICTH) is an uncommon but important clinical condition. It can occur in a setting of known malignancy. Here, we report the case of a 56-year-old, non-diabetic, female patient with unresectable malignant pleural mesothelioma who presented with unexplained recurrent hypoglycaemia. Surreptitious use of insulin or other hypoglycaemic agents were ruled out. Inves...
متن کاملNesidioblastosis coexisting with non-functioning islet cell tumour in an adult.
The most common cause of hyperinsulinaemic hypoglycaemia in adult is insulinoma. Although nesidioblastosis is a rare but well-recognized disorder of persistent hypoglycaemia in infants, it is extremely rare in adults. We present a case of a 59-year-old woman with small neuroendocrine tumour of the tail of the pancreas, diagnosed by CT scans and MRI, and hypoglycaemic syndrome. Laparoscopic dista...
